Job Description:
We seek a dedicated and compassionate Registered Nurse (RN) to join our home health and hospice care team. In this role, you will be at the forefront of delivering high-quality care to our patients, including end-of-life care for our hospice patients. You will work closely with patients, their families, and other healthcare professionals to create and implement comprehensive care plans that meet each patient's needs.
Responsibilities:
- Conduct initial assessments of new patients and develop individualized care plans.
- Provide direct patient care by the patient's individualized care plan.
- Provide skilled nursing care, including medication administration, wound care, and monitoring of vital signs.
- Collaborate with a multidisciplinary team to facilitate integrated and comprehensive care.
- Assess, monitor, document, and report the progress of the patient's health and condition.
- Provide emotional support and compassionate care to patients and their families.
- Educate patients and their families on disease management, self-care, and end-of-life planning.
- Coordinate with medical social workers and chaplain to provide bereavement support to families.
- Maintain accurate and timely documentation of patient care.
- Adhere to all company policies, procedures, and regulatory requirements.
Qualifications:
- Current and unencumbered RN or LPN license in the state of New Mexico.
- Excellent communication, interpersonal, and organizational skills.
- Ability to work independently and manage time effectively.
- Basic computer skills, including experience with electronic medical records (EMR) systems.
- CPR certification and valid driver's license with reliable transportation.
- Strong problem-solving and decision-making skills.
- Compassionate and empathetic demeanor with a strong dedication to patient care.
