What are the responsibilities and job description for the Clinical Social Worker position at Baton Rouge Behavioral Hospital?
The Social Worker is responsible for providing quality care management and psychotherapeutic services to adults chemically dependent and/or psychiatric disorder (age range approximately 18 and up). The Social Worker serves as a member of the multi-disciplinary team, supporting the facility’s treatment program and philosophy, and assuring the deliverance of quality treatment to adult patients and their families. Serves actively and constructively as a member of the multi-disciplinary team. Strong organizational skills and internal drive and motivation are necessary for success in this position. Competitive pay commensurate with experience. Interpersonal skills, judgment, and commitment are essential to the position.
RESPONSIBILITIES
· Attends mandatory training in Safety/Risk Management, CPR, infection Control, CPI, Exposure Control and HIV/Aids.
· Conducts psychosocial assessments to document history and identify preliminary issues for the treatment focus, by interviewing the patient and their family members. Communicates pertinent findings to the treatment team.
· Provides group and family process-oriented therapies using professional treatment modalities.
· Provide individual therapy as needed per treatment team request.
· Develops and coordinates an individualized discharge plan for the patient by utilizing treatment team and written chart information to determine the patient’s aftercare needs. Access by phone or written correspondence family or community resources such as nursing homes, group homes, or private mental health practitioners to meet patient’s aftercare needs.
· Attends regular treatment team meetings to provide social work perspective in total case management of the patient by reading and discussing written progress notes from charts and communicating, and state or local agency legal requirements for case management with the multi-disciplinary team. Monitors and records the patient’s progress toward meeting goals and objectives.
· Conducts educational and other didactic groups for patients and/or family members.
· Participates in patient premature departure intervention with specific other specific treatment team members.
· Makes handwritten entries to patient charts describing therapy performed, the progress in the discharge planning process, or other intervention
· Completes post-discharge 30-day follow-up documentation.
· Reports incidents of suspected abuse using proscribed forms and protocols from the relevant community/state agency.
· Develops and delivers community education lectures to enhance community awareness of mental health issues and resources available to address these issues. Supports the hospital’s strategic marketing effort.
· Responds to psychiatric emergency codes utilizing training in the management of aggressive behavior.
· Participates in training and development for purpose of professional growth and skill enhancement.
· Provide back-up services for incoming assessment and referral calls.
· Support hospital’s safety and security program.
· Performs all other duties as assigned.
Education:
Licensed Social Worker (LSW)
Work Experience:
· Experience in an inpatient settings preferred.
· Attention to details and organization skills are necessary.
· Customer Service related experience a plus.
· Excellent written and verbal communication skills.
· Must be able to speak and comprehend English fluently.
· Demonstrate experience and skills in case management, marital and family group therapy, time management, and crises intervention common to acute patients, as well as to non-violent crises intervention practices.
· Familiarity with follow-up resources in local communities to facilitate discharge and aftercare plan.
· Ability to clearly summarize pertinent clinical information via written correspondence and medical records documentation.
· Willingness to work flexible hours including weekends and evenings.
· Ability to work effectively with all facility staff to ensure a spirit of harmony, cooperation, and excellence in employee satisfaction.
· In the absence of these qualifications upon hire, must demonstrate ability to learn these concepts and develop these skills within a reasonable time frame.
