General Summary of Duties: We’re looking to add an LPN to our growing team of medical professionals. Your primary responsibility will be providing a variety of patient care activities and supportive services for the medical staff. Duties will include but are not limited to escorting patients, recording vital signs, and assisting practitioners during exams. Also, responsibilities entail ensuring patient confidentiality, participating in outreach, educating patients, and adhering to clinical protocol.
Typical Working Conditions: Full-time, inner-city medical office practice. Requires prolonged standing, sitting, bending, stooping, and lifting.
Duties and Responsibilities:
- Escort patients to exam rooms, take and record vital signs, weights, obtain an tests urine specimens and capillary blood sugars and do appropriate screenings as necessary in the prescribed manner.
- Assist practitioners with exams and/or treatments.
- Clean and prepare rooms between patients.
- Draw blood and do EKG's and other procedures as directed.
- Give allergy shots and other immunizations.
- Do nursing assessments and medical histories.
- Build relationships with patients.
- Triage and respond to phone messages in a timely fashion with correlated charting.
- Schedule patients as necessary.
- Coordinate schedule changes and "squeeze ins" with practitioners.
- Respond to emergency phone calls, calls from other doctors, and other appropriate calls from labs, specialists etc.
- Assess "walk-in" patients and decide the necessary course of action.
- Call in prescriptions and refills as required.
- Enter information into LINKS and handle consults/ referrals/enabling service orders as prescribed.
- Fill out patient forms in a timely and accurate manner including WIC forms, immunization records, physical exam forms and others.
- Maintains strict confidentiality and professionalism at all times regarding organizational business, patient information, staff, board and other potentially sensitive information and complies with all HIPPA requirements.
- Attend required meetings, in-services, and participate in committees as necessary.
- Perform other duties as assigned i.e.: immunization count, ordering quality controls, follow-up with abnormal paps, inventory and order medications and patient educational material.
- Participate in outreach activities as assigned.
- Obtain appropriate approval codes for appropriate procedures from Bayou Health and other Commercial Insurance Plans.
- Review lab reports and brings abnormal results to the attention of the practitioner.
- Check and record the result of previous day's cultures and follow up with appropriate measures.
- Schedule patient’s hospital tests, admissions, and visits to specialists.
- Educate patients on various health related topics and answers questions in a knowledgeable fashion.
- Assist other staff as appropriate.
- Maintain work area and assist in the general upkeep of the office.
- Follow clinical protocols and /office policy and procedures.

Education/Experience:
- Licensed LPN and current State Certification.
- Experience in hospital or outpatient setting
- Knowledge of the following required: Policies and procedures, prioritization of patient needs and patient flow, phone triage, practitioner support, nursing assessment, patient education, patient scheduling, referral procedures.
- Skills of the following required: Skills required to do phlebotomy, EKG's, injectable medications including allergy shots and immunizations, nursing assessment.
- Abilities one of the following required: Ability to communicate clearly in oral and written form. Ability to problem solve. Ability to organize and plan. Ability to be flexible and to work as part of a team in a spirit of cooperation.
Continuing Education/Competency Requirements:
- Completes all Required EHS training for position within 3 months of date of hire or change/update in Job Description
- Successful completion of a Clinical Competency Evaluation within 90 days of employment and annually thereafter
- Completes and provides evidence of Licensure Renewals annually and copies of Continuing Education Credits, ACLS/BLS Certification (Credentialing).
- Completes all Mandatory In-house Training (HIPPA, OSHA, Fire Safety) annually and any other mandated by the employer.
Physical Demands:
