SUMMARY: Responsible for facilitating patient discharges and transfers from the patient care areas. The Social Worker functions as coordinator of discharge plans, collaborating with the patient and caregiver, the patient’s care team, outside facilities and DME vendors, to ensure safe and timely patient discharges and transfers.
JOB REQUIREMENTS
Education: Bachelor’s Degree in Social Work (BSW) required.
Experience: Minimum two (2) years’ experience, either hospital or healthcare setting preferred.
Other: Working knowledge of Microsoft Office Suite, databases and general office equipment. Must be extremely detail oriented, organized and conscientious to ensure high quality work. Excellent time-management and follow-up skills to ensure work deadlines are met. Outstanding communication, presentation and interpersonal skills. Must be a positively contributing team player and able to maintain a high degree of confidentiality with regard to HIPAA and departmental operations.
Preferred: Master’s Degree in Social Work preferred.
