- SUMMARY : # The LPN, Clinic Case Manager, through patient assessment and engagement, chart review and collaboration with the clinic healthcare team, develops an appropriate care plan, making recommendations for alternative levels of care, identifying cost effective care options and facilitates appropriate clinical follow up. This work is done through patient assessment and engagement, chart review and collaboration with the healthcare team.# The LPN, Clinic Case Manager monitors patient progress throughout hospitalization or office visits, modifying plans as needed and continually assessing medical necessity. The LPN, Clinic Case Manager provides information to post hospital vendors as needed to ensure ongoing recovery for the patient. JOB REQUIREMENTS Education : Graduate of an accredited school of nursing. LPN Required.#### Licensure : Current Arkansas or Compact State Licensed Practical#Nurse license in good standing. Experience : Willing to train recent LPN graduate.# Previous Clinic experience desired. Certifications : BLS required. Other : Ability to read, write and communicate effectively. Basic computer knowledge required. Spends eight hours or more in front of computer, monitor or similar screen utilizing keyboard and / or mouse. Must be able to respond quickly in emergency situations of CPR and obtaining equipment. Must be able to handle highly stressful situations with efficiency and composure. Work Environment Clinic setting, within a hospital environment Position Type and Expected Hours of Work This is a position in a Clinical setting which is open 8-12 hours a day, up to 365 days a year. Holidays and work during inclement weather may be required.
