Description:


A Texas-based LVN role focusing on special needs adults, you'd provide focused nursing care, including administering medications and monitoring vital signs, under the direction of an RN, while collaborating with healthcare teams to implement individualized care plans.


Key Responsibilities:

  • Providing Basic Nursing Care:
  • Administering medications and monitoring vital signs.
  • Assisting with activities of daily living (ADLs) like bathing, dressing, and eating.
  • Providing comfort and support to patients.
  • Changing bandages and providing wound care.
  • Turning and rotating patient of 65 pounds
  • Moving patient from bed to wheelchair
  • Patient Assessment and Monitoring:
  • Collecting patient data, such as vital signs, and documenting observations.
  • Identifying and reporting any changes in a patient's condition to the RN.
  • Assessing patient needs and collaborating with the healthcare team to develop and implement individualized care plans.
  • Documentation and Record Keeping:
  • Maintaining accurate and thorough patient records.
  • Documenting medications administered, interventions performed, and patient responses.
  • Collaboration and Communication:
  • Communicating effectively with patients, families, and other healthcare professionals.
  • Participating in care conferences and team meetings.
  • Educating patients and families about their care plan and medications.
  • Other Duties:
  • Assisting with triage and discharge planning.
  • Maintaining a safe and clean environment for patients.

Experience:

  • While not always required, experience working with adults with special needs can be an asset.

Specific Considerations for Special Needs Adults:

  • Individualized Care Plans:

LVNs in this role must be able to implement individualized care plans that address the unique needs of each patient.

  • Communication Skills:

Strong communication skills are essential for interacting with patients who may have difficulty communicating their needs.

  • Patience and Compassion:

Working with individuals with special needs requires patience, empathy, and a compassionate approach.

  • Understanding of Disabilities:

Knowledge of various disabilities and their impact on daily living can be helpful.

Requirements:


  • Education and Licensure:
  • Graduation from an accredited LVN program.
  • Current licensure as a Licensed Vocational Nurse in the State of Texas.
  • Skills and Abilities:
  • Strong knowledge of basic nursing principles and practices.
  • Ability to administer medications and perform basic nursing procedures.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Ability to prioritize tasks and manage time effectively.
  • Knowledge of emergency procedures and CPR certification.
