What are the responsibilities and job description for the Community Health Representative position at Bay Mills Health Center?
POSITION SUMMARY:
To provide quality outreach health care services and health promotion/disease prevention services to patients of Bay Mills Health Center.
ESSENTIAL FUNCTIONS:
- To act as a liaison for the health care team to provide in-home check-ins. Must be able to perform duties within their scope.
- To provide transportation or routine, non-emergency appointments, for a patient without other means of transportation, when necessary.
- May provide medication deliveries
- Must be able to use software systems to document patient activity.
- Must be pleasant and respectful to BMHC patient population.
- To provide and/or assist in demonstrations, conduct training sessions and community meetings in the areas of safety, nutrition/dietetics, environmental health, and in other areas of health concerns.
- Maintain confidential files containing client information, vouchers, referrals and other related files in accordance with HIPAA.
- Must attend all mandatory or departmental training offered by the Bay Mills Human Resource Department and the Health and Human Services Program.
- Other duties may be assigned within the scope and complexity of this position’s essential functions.
PHYSICAL REQUIREMENTS:
While performing the duties of this job, the employee may be required to sit, stand, walk, bend, squat, climb, crouch, kneel, push/pull, and lift or move up to 50 pounds. Some of the work performed is in an office setting with adequate lighting, heating, and ventilation. The employee must be able to get in and out of vehicles regularly and assist patients in and out of vehicle if required.
POSITION REQUIREMENTS:
- High school diploma or G.E.D.
- Experience in health-related fields preferred, but not necessary.
- Must have a valid Michigan Driver’s License and auto insurance without restrictions.
- Must have dependable motor vehicle and be able to travel. Travel may be required overnight when needed.
- Must attend CHR training for a period of three weeks.
- Must have an excellent past work record including attendance as demonstrated through past personal and professional references.
- Must have excellent communication skills: both written and verbal.
- Must be drug free and participate in random drug testing, including but not limited to cannabis.
- To perform this position successfully, an individual must be able to satisfactorily perform each function listed under the essential functions and physical demands categories of this position description.
HIGH_SCHOOL
WORK SCHEDULE:
Full-time; 52 weeks per year. In addition to regular business hours, the person in this position must be flexible around the needs of the employer, which may include occasional evening and/or weekend hours or occasional travel for employer business.
