Requirements for MDS Nurse Coordinator (RN/ LPN):
- Experience in MDS completion required
- Certification as a Resident Assessment Coordinator (RAC-CT) preferred
- Ability to monitor, evaluate, and manage plans of care for in-house residents and patients
- Excellent communication skills with residents, families and the interdisciplinary team
- Strong computer skills, including Microsoft Office, EMR systems, etc.
- 1-2 years experience in long term care
- Active, unencumbered licensure as a registered nurse (RN) or licensed practical nurse (LPN) in the state of MA
- Prior experience with PointClickCare (PCC) required.
Job Duties for MDS Nurse Coordinator (RN/ LPN)
- Assess and monitor proper treatment for residents in nursing homes.
- Determine the health status, care plans, and procedures for intake of residents according to state and federal standards.
- Perform clinical assessments
