BAYADA Home Health Care is currently seeking an Admissions Coordinator to join our Brattleboro, VT home hospice office.

Responsibilities for an Admissions Coordinator:
- Ensures that all activities performed align with the leadership of the Hospice team.
- Serves as the lead coordinator of patient pre-admission activities and is responsible for all aspects associated with the referral management processing of Hospice referrals from physicians, hospitals, facilities, and other community sources.
- Coordinates BAYADA Hospice services with referral sources, insurance companies, patients, and families.
- Processes patient referrals, coordinates the admission of patients, and collaborates closely with hospice branch operations and sales team.
- Enters patient admission data after all requirements of hospice have been met and documented.
- Collaborates with different BAYADA functional offices at to obtain necessary billing information, clinical information, referral source/account preferences, and builds effective working relationships within the work group for the benefit of our patients.
- Contacts Insurance company for eligibility and authorization. Confirms Hospice Service Line by obtaining sufficient client and insurance information.
Qualifications for an Admissions Coordinator:
- High School Graduate or GED required. Bachelor’s degree is preferred.
- Minimum two (2) years of experience and/or training with hospice.
- Experience in a medical office environment, customer service role, intake or inside sales experience is preferred.
- Proficient in Microsoft Office Suite (including Excel and Teams).
- Positive attitude and willingness to learn throughout employment.
- Self-starter with excellent verbal and written communication skills.
