Job Summary:
BAYADA Home Health Care is a leading provider of home health care services. We are seeking a skilled Registered Nurse to join our team as a Care Coordinator. This role will involve providing exceptional medical care to adult and geriatric clients living in their homes.
About the Role
The successful candidate will be responsible for following a designated care plan in accordance with the patient's needs. This includes making home visits to clients in designated geographic territories, performing assigned duties such as administration of medication, wound care, treatments, and procedures, monitoring clients' conditions and reporting changes to Clinical or Client Services Manager, and accurately documenting observations, interventions, and evaluations pertaining to client care management and services provided.
Key Responsibilities
- Develop and implement individualized care plans to meet the unique needs of each client
- Conduct thorough assessments of clients' physical and emotional well-being
- Provide education and training to clients and their families on disease management and self-care techniques
- Maintain accurate and detailed records of client interactions and progress
Requirements
- A current license as a Registered Nurse in Massachusetts
- Graduation from an accredited and approved nursing program
