- This is a full-time opportunity with ongoing assignments, providing clients with rehabilitation care and skilled nursing services.
- Prior home care experience as a Speech Language Pathologist is a plus but not required.
The ideal candidate will have excellent assessment, diagnosis, and intervention skills, with the ability to work independently and as part of a team.
You will be responsible for developing a treatment plan based on the evaluation, physician's orders, and recommended goals, and accurately documenting observations, interventions, and evaluations related to patient care management and services provided.
