Job Summary
BAYADA Home Health Care is seeking a skilled Physical Therapist to provide high-quality patient care in the comfort of clients' own homes. As a Physical Therapist, you will work closely with clients, families, and other healthcare professionals to develop and implement individualized care plans that promote independence, safety, and well-being.
Responsibilities:
- Conduct thorough assessments of clients' physical abilities and needs
- Develop and implement personalized care plans to address clients' goals and objectives
- Provide one-on-one therapy sessions to clients in their homes
- Communicate effectively with clients, families, and other healthcare professionals to ensure seamless care transitions
- Document all interactions, treatments, and progress updates in a timely and accurate manner
Requirements:
- Current licensure as a Physical Therapist in the state of Pennsylvania
- Minimum of one year of recent experience as a Physical Therapist
- Ability to work independently and manage time effectively
- Strong interpersonal, organizational, and problem-solving skills
- Solid computer skills; prior experience with electronic medical records (EMR) preferred
