About the Job
- This is a per-diem position that offers flexibility and autonomy, allowing you to choose when and how you work.

Responsibilities:
- Assess patient needs and develop individualized care plans in collaboration with licensed Physical Therapists.
- Provide direct patient care, including therapy sessions, education, and support.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
Requirements:
- CURRENT licensure or certification in the state of New Jersey.
- Graduation from a two-year college-level program in Physical Therapy Assistance or equivalent experience.
- At least one year's experience working in a healthcare setting, preferably in a home health care environment.
