About the Job
As a Home Health Physical Therapy Assistant (PTA), you will have the opportunity to work closely with our experienced interdisciplinary team to deliver comprehensive patient care in the comfort of our patients' homes. You will be responsible for developing and implementing individualized care plans that address the unique needs and goals of each patient.
Your Responsibilities:
- Conduct thorough assessments of patients to identify their strengths, weaknesses, and areas for improvement.
- Develop and implement effective care plans that address the physical, emotional, and social needs of patients.
- Communicate regularly with patients, families, and healthcare teams to ensure seamless coordination of care.
Requirements:
- Current licensure or certification as a Physical Therapy Assistant (PTA).
- Graduation from an accredited Physical Therapy Assistant program.
