Key Responsibilities:
- Follow a designated care plan in accordance with patient needs
- Perform assigned duties, including medication administration, wound care, treatments, and procedures
- Accurately document observations, interventions, and evaluations pertaining to client care management and services provided
A minimum of one year of recent clinical experience is required, with prior home care experience strongly preferred. Solid computer skills and experience with electronic medical records are also desired.
