Responsibilities:
- Develop and implement individualized care plans to promote optimal health outcomes for patients.
- Coordinate and facilitate communication among healthcare teams to ensure seamless care delivery.
- Monitor patient progress and make adjustments to care plans as needed.
- Document all patient interactions and progress in accordance with company policies.
Requirements:
- A current license as a Registered Nurse in Pennsylvania.
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
- Prior home care experience strongly preferred, but not required.
