Job Description
BAYADA Home Health Care is a leading home health care provider dedicated to delivering exceptional patient care and supporting the growth and development of our employees. We are seeking an experienced Registered Nurse to join our team in Audubon, New Jersey.
Key Responsibilities:
- Develop and implement comprehensive care plans tailored to each patient's needs
- Conduct home visits to clients in designated geographic territories, providing personalized care and support
- Administer medications, perform wound care, treatments, and procedures as required
- Monitor patients' conditions, reporting changes to Clinical or Client Services Manager
- Execute and document doctors' orders accurately
- Perform client assessments as necessary, identifying areas for improvement and developing strategies for enhanced care delivery
- Collaborate with interdisciplinary teams to ensure seamless care coordination
Requirements:
- A current license as a Registered Nurse in NJ
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience
- Prior home care experience strongly preferred, but not required
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
- Ability to work independently and manage time effectively
- Strong interpersonal skills
- Solid computer skills; prior experience with electronic medical records (EMR) preferred
- Ability to travel to cases as assigned
