Job Overview
We are seeking a skilled Clinical Care Specialist to provide high-quality care to our clients in their homes. As a licensed Physical Therapist, you will be responsible for performing home visits, assessing client needs, and developing individualized care plans.
Responsibilities
- Conduct thorough assessments of clients' physical abilities and create personalized care plans.
- Develop and implement effective interventions to improve clients' mobility, strength, and overall quality of life.
- Educate clients and their families on rehabilitative care techniques and promote independent living.
- Accurately document client progress and services provided.
Qualifications
- A current PA Physical Therapist license.
- A minimum of one year of recent work experience as a Physical Therapist.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
