Responsibilities
- Make home visits to clients, assessing their conditions and developing personalized care plans
- Perform various medical procedures, including wound care, medication administration, and treatments

- Maintain accurate records of patient interactions, progress, and any changes in condition
- Participate in ongoing education and training to stay up-to-date on best practices and new developments in the field
Requirements
- A current license as a Registered Nurse (RN) in New Hampshire
- Minimum one year of recent, verifiable clinical experience, preferably in home care or hospice
- Prior experience with electronic medical records (EMR) is highly desirable
- Strong communication and interpersonal skills, with the ability to work effectively with diverse populations
