Job Summary:
We are seeking an experienced Physical Therapist to join our team as a Home Health Therapist. As a member of our clinical staff, you will be responsible for providing skilled physical therapy services to patients in their homes, promoting independence and improving quality of life.

Responsibilities:

• Conduct thorough assessments of patients' physical abilities and develop personalized care plans to address their needs.
• Provide one-on-one therapy sessions, utilizing a range of techniques and interventions to promote strength, mobility, and function.
• Collaborate with other healthcare professionals to ensure seamless care coordination and optimal patient outcomes.
• Document patient progress and adjust treatment plans as needed to achieve the best possible results.

Requirements:
To succeed in this role, you will need:

• A current NJ Physical Therapist license
• A minimum of one year of recent work experience as a Physical Therapist
• Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
• Strong interpersonal and communication skills, with the ability to work effectively with patients, families, and healthcare teams
• Excellent time management and organizational skills, with the ability to prioritize tasks and manage multiple responsibilities
