Responsibilities
- Make home visits to assess clients' needs and develop personalized care plans.
- Perform diagnostic tests and measurements to determine the best course of treatment.
- Develop and implement individualized care plans that meet the unique needs of each client.
- Educate clients and family members on rehabilitative care and activities necessary for independent living.
Qualifications
- A current MD Physical Therapist license.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
