About the Role
We are seeking a compassionate and skilled Home Health Registered Nurse to join our team. As a Home Health Registered Nurse, you will perform home health visits in Southwest Missouri, providing care to clients recovering from an injury, illness, surgery, or hospitalization.
This is a full-time position offering true work-life balance with flexible schedules, including full-time, part-time, and per diem options.
- You will assess patients' needs, develop personalized care plans, and deliver highly skilled care, including medication management, wound care, and education.
- Progress your clients from discharge to meeting their goals at home, ensuring they receive the highest quality care.
Responsibilities
- Case manage, develop care plans, and deliver skilled care to patients in their own homes.
- Monitor patients' progress, adjusting care plans as needed to ensure optimal outcomes.
- Provide education to patients and/or their families on disease management, treatment plans, and healthy living.
Requirements
- Graduation from an accredited and approved nursing program.
- A current license as a Registered Nurse in Missouri.
