Job Description
We are seeking a Registered Nurse to provide home health visits throughout Schuylkill County.
- This is an ongoing opportunity for a motivated and compassionate nurse to make a difference in the lives of our clients.
Responsibilities
- The successful candidate will follow a designated care plan in accordance with patient needs.
- They will make home visits to clients in designated geographic territories.
- The nurse will perform assigned duties, including administration of medication, wound care, treatments, and procedures.
- Monitoring clients' conditions and reporting changes to Clinical or Client Services Manager is also a key responsibility.
- Following up with, executing, and properly documenting doctors' orders is essential.
- The nurse will perform client assessments as necessary.
- Case management and coordination are also critical components of this role.
- Accurately documenting observations, interventions, and evaluations pertaining to client care management and services provided is crucial.
Qualifications
- A current license as a Registered Nurse in Pennsylvania is required.
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience is necessary.
- Prior home care experience is strongly preferred but not required.
- Graduation from an accredited and approved nursing program is a must.
- The ideal candidate will be able to work independently and manage time effectively.
- Strong interpersonal skills and solid computer skills are also required.
