- Provide high-quality, patient-centered care to clients in the comfort of their own homes
- Develop and implement effective care plans, incorporating input from family members and healthcare teams
- Communicate clearly and effectively with clients, families, and healthcare teams to ensure seamless care coordination
- Document all client interactions and medical information accurately and thoroughly
Requirements and Qualifications
To be considered for this role, you must have:
- A current RN license in the state of Massachusetts
- At least 1 year of experience in home health care or a related field
- Excellent communication and interpersonal skills
- Ability to work flexible hours, including evenings and weekends
