About the Position
- Develop and implement individualized care plans that address clients' unique needs and goals.
- Conduct home visits to assess clients' progress and make adjustments to their care plans as needed.
- Collaborate with other healthcare professionals, including nurses and therapists, to ensure seamless care coordination.
- Maintain accurate records of client interactions and progress, using state-of-the-art electronic medical records (EMRs).
Requirements
- A current Pennsylvania Physical Therapist license.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- At least 2 years of experience as a Physical Therapist, preferably in a home health care setting.
