- Conduct thorough assessments, including mobility and strength evaluations, balance testing, and safety assessments.
- Develop and implement comprehensive care plans tailored to each client's unique needs.
- Collaborate with nursing staff to ensure seamless care coordination and effective communication.
- Educate clients and their families on wound prevention and management techniques.
- Accurately document all interactions and interventions in our electronic medical record system.
Requirements:
- Active licensure as a Physical Therapist in New Jersey.
- At least one year of recent experience in physical therapy, preferably in home health or geriatrics.
- Excellent communication and interpersonal skills, with ability to work independently and manage time effectively.
- Strong knowledge of rehabilitation principles and practices.
- Experience with electronic medical records and other software applications a plus.
