Responsibilities
- Assess and evaluate clients' physical function and mobility to develop effective care plans.
- Provide expert physical therapy services, including manual therapeutic exercises, gait training, balance and other interventions.
- Continually assess and revise care plans as needed to ensure optimal outcomes.
- Educate and instruct clients and family members on rehabilitative care and activities necessary to promote independence and safety.
- Accurately document all observations, interventions, and evaluations related to client care management and services provided.
