Job Summary
We are seeking an experienced Physical Therapist to join our team in providing home health care services to adults and geriatric clients. As a member of our multidisciplinary team, you will work closely with clients, families, and healthcare professionals to develop and implement individualized care plans.
Key Responsibilities
* Make home visits to clients in designated geographic territories
* Perform diagnostic tests and measurements, such as mobility assessments, strength evaluations, and balance testing
* Develop and implement individualized care plans, including manual therapeutic exercises, gait training, and other interventions
* Continually assess and revise the care plan, participating in multidisciplinary care planning as needed
* Educate and instruct clients, family members, or representatives on rehabilitative care and activities necessary for independent living
* Accurately document observations, interventions, and evaluations pertaining to client care management and services provided
Requirements
* Current NJ Physical Therapist license
* A minimum of one year of recent work experience as a Physical Therapist
* Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
* Ability to work independently and manage time effectively
* Strong interpersonal, organizational, and problem-solving skills
* Solid computer skills; prior experience with electronic medical records (EMR) preferred
