This position requires a highly motivated and compassionate Physical Therapist who can work independently and collaboratively as part of a multidisciplinary team. You will be responsible for conducting comprehensive assessments, developing individualized care plans, and implementing evidence-based rehabilitation programs to promote client recovery and independence.
Salary: Competitive compensation package, including $1,000 Sign On Bonus.
About the Role
- This position involves making outpatient visits to clients in designated geographic territories, assessing their physical abilities, and developing individualized care plans to promote their independence and quality of life.
- You will work closely with our nursing team to ensure seamless coordination of care and optimal client outcomes.
Responsibilities
- Conduct comprehensive assessments of clients' physical function and mobility, identifying areas for improvement and developing targeted interventions to address these needs.
- Design and implement evidence-based rehabilitation programs, incorporating manual therapeutic exercises, gait training, balance, and other interventions tailored to meet the unique needs of each client.
- Educate and empower clients, their families, and caregivers to take an active role in their care and recovery, promoting self-management and independence.
- Collaborate with healthcare professionals from various disciplines to coordinate care and optimize client outcomes, ensuring a cohesive and patient-centered approach.
Requirements
- A current PA Physical Therapist license and graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with clients, families, and colleagues.
- Ability to work independently and manage time effectively, prioritizing tasks and managing multiple responsibilities.
- Strong problem-solving and critical thinking skills, with a commitment to continuous learning and professional development.
