Key Responsibilities:
- Make home visits to clients in designated geographic territories, assessing their physical needs and developing personalized care plans.
- Perform diagnostic tests and measurements, including mobility assessments, strength evaluations, and balance testing.
- Develop and implement appropriate individualized care plans, incorporating manual therapeutic exercises, gait training, and other interventions as necessary.
- Continually assess and revise the Physical Therapy care plan, collaborating with nursing staff to ensure seamless multidisciplinary care.
- Educate and instruct clients, family members, or other client representatives on rehabilitative care and activities necessary to promote independent living.
- Accurately document observations, interventions, and evaluations utilizing state-of-the-art technology.
Qualifications:
- A current New Hampshire Physical Therapist license.
- A minimum of one year of recent work experience as a Physical Therapist.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills; prior experience with electronic medical records preferred.
