Expert Home Health Care Professional

Key Responsibilities:

• Make home visits to clients in designated geographic territories, assessing their physical needs and developing personalized care plans.
• Perform diagnostic tests and measurements, including mobility assessments, strength evaluations, and balance testing.
• Develop and implement appropriate individualized care plans, incorporating manual therapeutic exercises, gait training, and other interventions as necessary.
• Continually assess and revise the Physical Therapy care plan, collaborating with nursing staff to ensure seamless multidisciplinary care.
• Educate and instruct clients, family members, or other client representatives on rehabilitative care and activities necessary to promote independent living.
• Accurately document observations, interventions, and evaluations utilizing state-of-the-art technology.

Qualifications:

• A current New Hampshire Physical Therapist license.
• A minimum of one year of recent work experience as a Physical Therapist.
• Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; prior experience with electronic medical records preferred.
