BAYADA Home Health Care is seeking a highly skilled and dedicated Home Health Physical Therapist to join our team in Northfield, New Jersey.
About the Role
This full-time or part-time opportunity involves performing home health visits for adult and geriatric clients in Atlantic county. As a Home Health Physical Therapist, you will be an integral member of a multi-disciplinary healthcare team providing rehabilitative care and skilled nursing services to clients, enabling them to receive medical care at home.
Responsibilities:
- Perform diagnostic tests and measurements, including mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
- Develop and implement individualized care plans, incorporating manual therapeutic exercises, gait training, balance, and other interventions.
- Continually assess and revise the Physical Therapy care plan, participating with nursing in the multidisciplinary care plan as needed.
- Educate and instruct clients, family members, or client representatives on rehabilitative care and activities promoting client health, safety, and independent living.
- Accurately document observations, interventions, and evaluations related to client care management and services provided using state-of-the-art touch pad tablets.
Requirements:
- A current New Jersey Physical Therapist license.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
