We are seeking a compassionate and skilled Home Health Physical Therapist to join our team in Glen Ridge, New Jersey.
About the Role:
This is a full-time opportunity to provide home health visits for adult and geriatric clients in the Essex County area. As a member of our multi-disciplinary healthcare team, you will play a crucial role in delivering rehabilitative care and skilled nursing services that enable clients to remain at home.
Responsibilities:
- Perform diagnostic tests and measurements, such as assessing mobility, range of motion, strength, and balance.
- Develop and implement individualized care plans, including manual therapeutic exercises, gait training, and other interventions.
- Educate clients and their families on rehabilitative care and activities necessary for independent living.
- Accurately document client care management and services provided using electronic medical records (EMR).
Qualifications:
- A current New Jersey Physical Therapist license.
- A minimum of one year of recent work experience as a Physical Therapist.
- Graduation from an accredited physical therapy program.
- Solid computer skills and prior experience with EMR systems.
