Home Health Physical Therapist
We are seeking a compassionate and skilled Physical Therapist to join our team as a home health physical therapist in Port Carbon, Pennsylvania.
About the Role:
- Perform diagnostic tests and measurements to assess clients' mobility and strength.
- Develop and implement individualized care plans to promote clients' health and independence.
- Educate and instruct clients and their families on rehabilitative care and activities.
- Accurately document client care management and services provided.
Qualifications:
- A current Pennsylvania Physical Therapist license.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Solid computer skills and prior experience with electronic medical records (EMR) preferred.
