As a Physical Therapist on our team, you will be an integral member of a multi-disciplinary healthcare team that provides skilled nursing and rehabilitative care to clients in their homes and senior living communities.
Responsibilities and Expectations
- Making home care visits to clients in Senior Living Communities and in their homes.
- Conducting diagnostic tests and measurements, such as mobility, range of joints, transfer status, stability, patterns, and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
- Developing and implementing individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
- Continually assessing and revising the physical therapy care plan and participating with nursing in the multidisciplinary care plan.
- Educating and instructing clients, family members, or other client representatives in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
- Accurately documenting observations, interventions, and evaluations pertaining to client care management and services provided.
Requirements and Qualifications
- A current license as a Physical Therapist in Massachusetts.
- Recent clinical experience as a licensed Physical Therapist.
- Ability to travel to cases as assigned.
