BAYADA Home Health Care is seeking a dedicated and compassionate Physical Therapist to join our team. As a Physical Therapist Home Care Specialist, you will provide home-based physical therapy services to adult and geriatric clients, promoting independence, mobility, and overall well-being.
Responsibilities include assessing clients' physical abilities, developing and implementing individualized care plans, and providing manual therapies, exercise programs, and patient education. You will also collaborate with our multidisciplinary healthcare team to ensure coordinated care and optimal outcomes.
Required Skills and Qualifications:
- Graduation from an accredited Physical Therapy program
- Current licensure as a Physical Therapist in the state of Pennsylvania or New Jersey
- Experience working with geriatric clients and their families
- Strong communication and interpersonal skills, with the ability to work effectively with clients, families, and healthcare professionals
- Ability to work independently and manage time efficiently in a fast-paced environment
