Job Summary:
We are seeking an experienced Physical Therapist to join our team at BAYADA Home Health Care. As a member of our multidisciplinary healthcare team, you will work closely with clients, families, and caregivers to develop and implement individualized care plans that promote independence, safety, and overall well-being.
Key Responsibilities:
- Conduct comprehensive assessments of clients' physical function and mobility
- Develop and implement individualized care plans that address the unique needs of each client
- Collaborate with clients, families, and caregivers to educate them on the importance of rehabilitation and provide support throughout the recovery process
- Continuously assess and revise the care plan to ensure optimal outcomes and patient satisfaction
Qualifications:
- A current PA Physical Therapist license
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
- Ability to work independently and manage time effectively
- Strong interpersonal, organizational, and problem-solving skills
- Solid computer skills; prior experience with electronic medical records (EMR) preferred
