Geriatric Rehabilitation Therapist
We are seeking an experienced Physical Therapist to join our team in providing high-quality geriatric rehabilitation services to seniors in their homes. As a member of our multidisciplinary team, you will play a vital role in helping our clients maintain their independence and quality of life.
About the Job
This position involves working with clients in designated senior living communities, assessing their needs, developing and implementing individualized care plans, and educating them on therapy services and activities necessary for independent living. You will also be responsible for performing diagnostic tests and measurements, continually assessing and revising the physical therapy plan of care, and participating in multidisciplinary care plan meetings as appropriate.
Responsibilities
* Assess client needs and develop individualized care plans* Perform diagnostic tests and measurements, such as mobility and range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments* Implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions* Continually assess and revise the physical therapy plan of care, and participate in multidisciplinary care plan meetings as appropriate* Educate and instruct clients, family members, or community staff, in therapy services and activities necessary to promote the client's health, safety, and independent living
Requirements
* A current VA Physical Therapist license* Outpatient experience a plus but not required* Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association* Ability to work independently and manage time effectively* Dynamic and strong interpersonal, organizational, and problem-solving skills* Solid computer skills; prior experience with electronic medical records (EMR) preferred
