About the Role
We are seeking a dedicated and compassionate Home Health Speech Language Pathologist to join our team. As a member of our interdisciplinary team, you will have the opportunity to work with clients who require speech, language, communication, or swallowing therapy. You will be responsible for conducting thorough evaluations, developing and implementing individualized treatment plans, and providing one-on-one therapy sessions to clients in their homes.
Responsibilities
- Conduct thorough evaluations to assess and diagnose speech and language disorders
- Develop and implement individualized treatment plans based on client goals and needs
- Provide one-on-one therapy sessions to clients in their homes
- Communicate effectively with clients, families, and healthcare professionals to ensure seamless care coordination
- Maintain accurate and timely documentation of client progress and services provided
Requirements
- Certificate of Clinical Competence in Speech from the American Speech-Language-Hearing Association (ASHA)
- Current license as a Speech Language Pathologist in PA
- Solid computer skills, including experience with electronic medical records (EMR)
- Excellent communication and interpersonal skills
