Bayada Home Health Care is seeking a skilled Habilitation Manager to join our team in Pittsburgh, PA. As a Habilitation Manager, you will play a vital role in ensuring the delivery of high-quality client care services.
Key Responsibilities
- Collaborate with the Clinical Manager to act as internal case manager and advocate for clients in the Habilitation program.
- Participate in Person-Centered Planning processes to identify individual client goals and needs.
- Work closely with the client, family, Case Managers, Hab Techs, and other teams to implement and evaluate client care plans.
- Maintain ongoing communication with Case Managers/Care Coordinators to ensure seamless client care transitions.
- Carry out supervision of Habilitation Technicians according to policy and procedure.
Qualifications
- Bachelor's degree required.
- Must be a Qualified Professional with minimum years of post-degree experience working with individuals with intellectual/developmental disabilities.
- Prior supervisory experience is a plus.
- Demonstrated record of successfully taking on increased responsibility.
- Ambition to grow and advance beyond current position.
- Excellent communication and interpersonal skills.
