About the Position:

Responsibilities:
- Providing physical therapy services to patients in their homes, including assessments, goal setting, and treatment planning.
- Working closely with other healthcare professionals to develop and implement patient care plans.
- Communicating effectively with patients, families, and caregivers to ensure seamless care coordination.
Requirements:
To be successful in this role, you will need:
- A current licensure or certification in the state of New Jersey.
- Graduation from a two-year college level program approved by the American Physical Therapy Association.
- At least one year's work experience under the supervision of a qualified Physical Therapist.
