Job Description: As a Home Health Physical Therapist, you will be responsible for providing high-quality rehabilitative care to clients in their homes. This may involve developing and implementing individualized care plans, educating clients and family members on rehabilitative care and activities, and maintaining accurate records of client care management and services provided.
Responsibilities:
- Develop and implement individualized care plans to meet the unique needs of each client
- Provide education and support to clients and family members on rehabilitative care and activities
- Maintain accurate and up-to-date records of client care management and services provided
- Collaborate with other healthcare professionals to ensure seamless care transitions
Qualifications:
- A current New Hampshire Physical Therapist license
- A minimum of one year of recent work experience as a Physical Therapist
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
- Excellent communication and interpersonal skills
