BAYADA Home Health Care seeks a talented Occupational Therapist to join its home health care team.

About the Role

The ideal candidate will work collaboratively with a multidisciplinary team to deliver in-home rehabilitation services, prioritizing client-focused care and achieving optimal outcomes.

Key Responsibilities:

• Conduct thorough patient evaluations in senior living communities or group homes for adults with intellectual disabilities, developing individualized plans of care.
• Work closely with home health therapy partners to ensure seamless transitions from home health to outpatient services, maintaining continuity of care.
• Educate clients, families, and caregivers on developed care plans, empowering them to enhance functional independence.
• Perform assessments, treatments, and discharge planning for patients using an outpatient EMR platform.

Requirements:

• A valid New Jersey Occupational Therapist license.
• Prior home care experience is beneficial but not mandatory.
• A minimum of one year of recent experience as an Occupational Therapist.
• Completion of an occupational therapy program accredited by the Joint Committee on Allied Health Education and Accreditation.
• Proven ability to work autonomously and manage time efficiently.
• Excellent communication, organizational, and problem-solving skills.
• Proficiency in computer applications, particularly electronic medical records (EMR).
