About the Job
This full-time position involves delivering 1:1 skilled nursing, intervention, and supportive care to patients in their own homes. As a Jefferson Health at Home by BAYADA RN, you will work closely with clients to assess their needs, develop individualized care plans, and provide education to promote independence and comfort.
Responsibilities
- Perform comprehensive assessments of clients' physical and emotional needs
- Develop and implement personalized care plans that address clients' unique requirements
- Provide direct client care, including wound management, medication administration, and vital sign monitoring
- Maintain accurate records of client interactions, progress, and outcomes
To succeed in this role, you will possess excellent communication and interpersonal skills, as well as a calm and reassuring presence. You will be able to travel to cases as assigned and have access to reliable transportation.
Requirements
- Active and current license to practice as a professional registered nurse (RN) in the state of PA
- One year of clinical experience preferred
- Ability to work independently and as part of a larger care team
- Excellent assessment, critical thinking, and problem-solving skills
