About the Role
We are seeking a qualified Registered Nurse to join our team as a Home Health Care Specialist. As a key member of our healthcare team, you will provide high-quality patient care and support to clients in their homes.
Responsibilities include:
- Conducting home visits to assess client needs and develop personalized care plans
- Administering medication, wound care, treatments, and procedures as required
- Monitoring client conditions and reporting changes to Clinical or Client Services Manager
- Following up with and executing doctors' orders
- Performing client assessments and case management
Requirements
- A current license as a Registered Nurse in PA
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience
- Prior home care experience strongly preferred, but not required
- Graduation from an accredited and approved nursing program
