About the Role
This position involves providing palliative care to clients on a per-visit basis in territories throughout Gloucester, Salem, and Cumberland Counties. We are looking for an individual with a strong commitment to delivering high-quality patient care and a passion for working in a fast-paced environment.
The ideal candidate will have a current license as a Registered Nurse in NJ and a minimum of one year of recent, verifiable clinical (medical/surgical) experience. Prior home care experience is strongly preferred but not required.
Responsibilities
As a Medical Care Associate, you will be responsible for:
- Providing palliative care to clients in their homes
- Managing clients' medical needs and creating personalized care plans
- Communicating effectively with clients, families, and healthcare teams
- Documenting client information and progress accurately
