Responsibilities:
- Develop and implement individualized care plans to meet the unique needs of each client.
- Conduct thorough assessments to identify areas of improvement and create targeted interventions.
- Collaborate with other healthcare professionals to ensure seamless delivery of care.
- Provide education and training to clients and their families on proper care techniques and safety measures.
- Maintain accurate and detailed records of patient progress and outcomes.
Requirements:
- Current Physical Therapist license in Arizona.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Solid computer skills and prior experience with electronic medical records (EMR) preferred.
