We are seeking a skilled and compassionate Registered Nurse to join our Home Health team in Pennsylvania. As a key member of our team, you will be responsible for delivering high-quality patient care in the comfort of clients' homes.
Job Summary
In this rewarding role, you will work closely with our Clinical Managers to develop and implement personalized care plans that cater to the unique needs of each client. Your expertise will enable you to assess patients' conditions, administer medications, provide wound care, and perform various medical procedures as required.
Key Responsibilities
- Patient Assessment and Care Planning: Conduct thorough assessments of clients' physical and emotional needs to create comprehensive care plans.
- Medication Administration: Safely administer prescribed medications to clients, ensuring accurate dosages and adherence to treatment plans.
- Wound Care and Treatment: Provide specialized wound care and treatments, including dressing changes, debridement, and other interventions as needed.
- Client Monitoring and Reporting: Regularly monitor clients' conditions and report any changes or concerns to Clinical Managers.
- Documentation and Record-Keeping: Accurately document all interactions, observations, and interventions in clients' electronic medical records.
Requirements
- A current license as a Registered Nurse in Pennsylvania is mandatory.
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience is required.
- Prior home care experience is strongly preferred but not necessary.
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma, is essential.
- The ability to work independently and manage time effectively is crucial.
- Solid interpersonal skills are necessary for effective communication with clients, families, and healthcare teams.
- Familiarity with electronic medical records (EMR) systems is preferred.
- The ability to travel to cases as assigned is required.
