We are seeking a qualified Registered Nurse (RN) to provide high-quality home health care services in our community.
About the Position
As a Registered Nurse (RN), you will be responsible for providing patient-centered care to clients in their homes, ensuring that they receive the highest quality of care and support.
Key Responsibilities:
- Follow a designated care plan and make home visits to clients in designated geographic territories.
- Perform assigned duties, including administration of medication, wound care, treatments, and procedures.
- Monitor clients' conditions and report changes to Clinical or Client Services Manager.
- Accurately document observations, interventions, and evaluations pertaining to client care management and services provided.
Requirements:
- A current license as a Registered Nurse in PA.
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
- Prior home care experience is strongly preferred but not required.
