Job Description
We are seeking a skilled Physical Therapist to join our team of healthcare professionals at BAYADA Home Health Care.
About the Role:
- This position involves providing high-quality, patient-centered care to adults and geriatric clients in their homes throughout the Lake Sunapee area.
- The successful candidate will be responsible for conducting diagnostic tests, developing individualized care plans, and implementing therapeutic interventions to promote optimal health and function.
- As part of our multidisciplinary team, you will work collaboratively with nursing staff to provide comprehensive care and support to clients.
Responsibilities:
- Conduct home visits to assess client needs and develop personalized care plans.
- Perform diagnostic tests and measurements to inform treatment decisions.
- Implement manual therapeutic exercises, gait training, and other interventions to improve client mobility and function.
- Educate clients and their families on rehabilitative techniques and strategies to promote independent living.
- Accurately document observations, interventions, and evaluations to ensure seamless communication with the healthcare team.
Requirements:
- A current VT Physical Therapist license is required.
- A minimum of one year of recent work experience as a Physical Therapist is preferred.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association is mandatory.
- Strong interpersonal, organizational, and problem-solving skills are essential.
- Solid computer skills, including experience with electronic medical records (EMR), are highly valued.
