About the Role:
- As a Home Health Occupational Therapist, you will be responsible for providing rehabilitative care and skilled nursing to clients in their homes.
- You will work with a multidisciplinary healthcare team to develop individualized intervention plans and achieve favorable outcomes.
- Your primary focus will be on assessing patients' functional status, developing treatment plans, and educating patients and their families on necessary skills for independent living.
Responsibilities:
- Conduct thorough evaluations of patients' functional status, including muscle function, endurance, visual coordination, written and verbal communication skills, and ADL and IADL performance.
- Develop and implement individualized intervention plans to address cognitive, psychological, and motor aspects of performance.
- Evaluate home environments for hazards or barriers to independent living and identify equipment needs and environmental adaptations.
- Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided.
